Start with a White (Normal) Item
Find a base with the right implicit and item level. Higher ilvl = higher tier mods possible.

Transmutation → Alteration Loop
Use Orb of Transmutation to make it Magic, then Orb of Alteration to reroll magic modifiers until you hit desired affixes.

Regal Orb to Rare
Once you have a desirable magic mod, use a Regal Orb to upgrade to Rare, adding one new random modifier.

Exalted Orb for Extra Mods
Add additional modifiers to open affix slots on your rare item with Exalted Orbs.

Divine Orb to Perfect Values
Use Divine Orbs to reroll the numeric values of existing modifiers to maximum rolls.
